Understanding and implementing your NDIS plan can feel overwhelming. At High Support Care, our Support Coordination team is here to make it easier. We help you connect with the right providers, build your capacity to manage supports, and give you more choice and control over your services.
Whether you're new to the NDIS or looking to get more from your plan, our experienced coordinators will guide you every step of the way.
Support Coordination is a Capacity Building support funded under your NDIS plan. It helps you understand your plan, connect with service providers, and build the skills to manage your supports with greater independence over time.
Our coordinators work with you—and your family or carers if you choose—to ensure your funding is used effectively and your goals are being met.

We explain your NDIS plan in plain language—what's funded, what's flexible, and how to make the most of your budget. We help you set goals and align your supports with what matters to you.
We help you find and connect with service providers that match your needs, preferences, and location. We can assist with Service Agreements and ensure you're getting value from your supports.
Our goal is to help you become more confident in managing your own supports. We teach you how to navigate the NDIS, use the participant portal, and make informed decisions about your care.
When things get difficult, we're here to help. We can assist with resolving service issues, managing changes, and preparing for plan reviews so you're ready when the time comes.
The NDIS funds different levels of Support Coordination depending on your needs:
Support Connection: Helps you connect with informal, community, and funded supports. Lower intensity, designed to build your confidence.
Coordination of Supports: Broader support to connect with providers, build capacity, resolve points of crisis, and strengthen your ability to manage your plan.
Specialist Support Coordination: For participants with complex barriers or high needs. Involves specialist expertise and may include NDIS plan requirements or specific interventions.

Support Coordination is a Capacity Building support. It appears in your NDIS plan if the NDIS has assessed that you need help to connect with and manage your supports. If you're unsure whether you have it, check your plan or contact us—we can help you understand your funding.
Yes. High Support Care offers Support Coordination as well as a range of other NDIS supports. If you use us for Support Coordination, we can help you connect with our own services or other providers— whatever best meets your needs. Your coordinator will always act in your best interest.
